I'm guessing you've run into the problem of not being able to bind functions to your mouse buttons? When in the Controls menus, you will see 'Device'. Click that to add binds to your mouse buttons. Yes, it's sloppy, and you'd not be the first to overlook this.
FO3 is pretty sweet. There are many nice mods that don't change the gmeplay, like texture packs and such, but one mod I'd STRONGLY suggest any PC player uses is the "MTUI" mod. All it does is change teh interfaces a bit to be more PC-friendly. it's a relatively minor change, but it has a big impact, considering all the chat and other text lists you'll be poking through.

I have the Texture Pack, and the improved weapons pack. OK, I'll look again about the "Device", and see what comes up.
